Cumhuriyetin ilan edilmesinin ilk y\u0131llar\u0131nda itibaren \u00fclkemizdeki bug\u00fcnk\u00fc sa\u011fl\u0131k te\u015fkilatland\u0131rmas\u0131 ve sa\u011fl\u0131k hizmetlerinin temeli at\u0131lm\u0131\u015ft\u0131r. Cumhuriyet d\u00f6neminde bir yandan tedavi hizmet a\u011f\u0131n\u0131 geni\u015fletmek istenirken \u00f6teki yandan da koruyucu sa\u011fl\u0131k hizmetlerinde b\u00fcy\u00fck bir ata\u011fa ge\u00e7ilmi\u015ftir. \u00a0Cumhuriyet\u2019in ilan\u0131 sonras\u0131 Dr. Refik Saydam \u00fclkemizde sa\u011fl\u0131k hizmetlerinin kurulu\u015funda ve geli\u015fmesinde b\u00fcy\u00fck katk\u0131lar sa\u011flam\u0131\u015ftir. Umumi H\u0131fz\u0131s\u0131hha Kanunu (1930)\u2019nda yer alan sa\u011fl\u0131k politikalar\u0131 sa\u011fl\u0131k hizmetlerinin planlanmas\u0131 ve programlanmas\u0131, koruyucu ve tedavi edici hekimli\u011fin y\u00fcr\u00fct\u00fclmesi, bula\u015f\u0131c\u0131 hastal\u0131klar\u0131n \u00f6nlenmesi, t\u0131p fak\u00fcltelerine ilginin artt\u0131r\u0131lmas\u0131 y\u00f6nelik ilkeler mevcut olup bu ilkeler \u0131\u015f\u0131\u011f\u0131nda sa\u011fl\u0131k hizmetleri, \u201cgeni\u015f b\u00f6lgede tek ama\u00e7l\u0131\u201d\/ \u201cdikey \u00f6rg\u00fctleme\u201d modeli ile y\u00fcr\u00fct\u00fclm\u00fc\u015ft\u00fcr. N\u00fcfusun \u00e7ok oldu\u011fu yerlerden ba\u015flayarak muayene ve tedavi evleri a\u00e7\u0131lm\u0131\u015f, koruyucu sa\u011fl\u0131k hizmetlerinde \u00e7al\u0131\u015fan hekimler desteklenmi\u015f g\u00fcn\u00fcm\u00fcz sa\u011fl\u0131kta d\u00f6n\u00fc\u015f\u00fcm program\u0131n\u0131n temelleri daha o zamanlar da at\u0131lm\u0131\u015ft\u0131r.<\/p>\n<p>Sa\u011fl\u0131kta D\u00f6n\u00fc\u015f\u00fcm Program\u0131, temel sa\u011fl\u0131k hizmetlerinin kurumsal konumunu di\u011fer hizmet d\u00fczeyleri \u00fczerinde yetki ve kontrol sahibi olacak bir yap\u0131ya kavu\u015fturmay\u0131 hedeflemektedir. Bireylerin ve sa\u011fl\u0131k \u00e7al\u0131\u015fanlar\u0131n\u0131n durumlar\u0131n\u0131 iyile\u015ftirmek bu konuda yap\u0131lacak yeniliklerin hareket noktas\u0131n\u0131 olu\u015fturmaktad\u0131r. Sa\u011fl\u0131kta d\u00f6n\u00fc\u015f\u00fcm program\u0131n\u0131n en belirgin \u00f6zelli\u011fi bireylerin sa\u011fl\u0131kl\u0131 hayat programlar\u0131na eri\u015fiminin sa\u011flanmas\u0131, anne-bebek \u00f6l\u00fcmlerinin azalt\u0131lmas\u0131, bula\u015f\u0131c\u0131 hastal\u0131klarla ve kronik hastal\u0131klar\u0131n risk fakt\u00f6rleriyle m\u00fccadele etmeyi \u00f6ncelemek bireylerin kendi sa\u011fl\u0131k durumlar\u0131n\u0131 kontrol edebilme yetilerini geli\u015ftirmek ve koruyucu hekimlik yakla\u015f\u0131m\u0131n\u0131\u00a0 sa\u011fl\u0131\u011f\u0131n merkezine tamamen yerle\u015ftirmektir.<\/p>\n<p>Sa\u011fl\u0131kta d\u00f6n\u00fc\u015f\u00fcm program\u0131n\u0131n temel ama\u00e7lar\u0131 aras\u0131nda olan bu uygulaman\u0131n, halk\u0131m\u0131z\u0131n sa\u011fl\u0131k d\u00fczeyinin y\u00fckseltilmesi, kaynaklar\u0131m\u0131z\u0131 uygun \u015fekilde kullanarak daha fazla hizmetin sa\u011flanmas\u0131 ve hakkaniyet ilkesi \u0131\u015f\u0131\u011f\u0131nda b\u00fct\u00fcn bireylerin sa\u011fl\u0131k hizmetlerine ihtiya\u00e7lar\u0131 \u00f6l\u00e7\u00fcs\u00fcnde ula\u015fmalar\u0131n\u0131n gerek k\u0131r-kent ve gerekse do\u011fu-bat\u0131 aras\u0131nda sa\u011fl\u0131k hizmetlerine eri\u015fim ve sa\u011fl\u0131k g\u00f6stergeleri ile ilgili farkl\u0131l\u0131klar\u0131n azalt\u0131lmas\u0131\u00a0 yer almaktad\u0131r. Bu ama\u00e7lara ula\u015fmak i\u00e7in birinci basamak sa\u011fl\u0131k hizmetlerinin \u00e7a\u011fda\u015f bir yakla\u015f\u0131mla yeniden d\u00fczenlenip yayg\u0131nla\u015ft\u0131r\u0131lmas\u0131 ve t\u00fcm toplum bireyleri taraf\u0131ndan tercih edilebilir \u015fekilde sunulmas\u0131 sa\u011flanmaktad\u0131r. Herkesin kendi se\u00e7ebilece\u011fi, kolayca eri\u015febilece\u011fi, herhangi bir engelle kar\u015f\u0131la\u015fmas\u0131z\u0131n dan\u0131\u015fabilece\u011fi, ba\u015fvurabilece\u011fi bir aile hekiminin olmas\u0131 bu yakla\u015f\u0131m\u0131n ana unsurlar\u0131n\u0131n ba\u015f\u0131nda gelir. Prof. Dr. Nusret Fi\u015fek\u2019in ifadeleriyle: \u201c Ki\u015fiye y\u00f6nelik koruyucu hekimlik hizmetleri ile ayakta ve evde hasta tedavisi hizmetlerinin entegre y\u00fcr\u00fct\u00fclmelidir\u201d Entegre sa\u011fl\u0131k modeli, \u00e7a\u011fda\u015f aile hekimli\u011fidir.<\/p>\n<p>\u00c7a\u011fda\u015f aile hekimli\u011fi\u00a0 ile \u00fclkemizde sa\u011fl\u0131k hizmetlerinin t\u00fcm bireylere co\u011frafi a\u00e7\u0131dan dengeli \u015fekilde ula\u015ft\u0131r\u0131labilmesi, birinci basamak sa\u011fl\u0131k hizmetlerinin toplumun kat\u0131l\u0131m\u0131n\u0131 sa\u011flayacak \u015fekilde bireylerin ya\u015fad\u0131klar\u0131 ve \u00e7al\u0131\u015ft\u0131klar\u0131 yerlerde koruyucu, tan\u0131 koyucu tedavi ve rehabilite edici y\u00f6nleriyle sunmak temel hedefimizdir.\u00a0 Sa\u011fl\u0131k hizmeti sunumunda,\u00a0 birinci basamak sa\u011fl\u0131k hizmetlerinin\u00a0 s\u00fcrekli e\u011fitimle geli\u015ftirilmesi ve g\u00fc\u00e7lendirilmesi, \u00e7al\u0131\u015fan hekimler ile di\u011fer sa\u011fl\u0131k elemanlar\u0131n\u0131n \u00f6zendirilmesi, birey ihtiya\u00e7lar\u0131n\u0131n g\u00f6z \u00f6n\u00fcnde bulundurularak koruyucu sa\u011fl\u0131k sistemine a\u011f\u0131rl\u0131k verilmesi ve kabul edilebilir sevk sisteminin uygulanmas\u0131 ana ilkelerdir. Bu ilkeler ikinci basamakta y\u0131\u011f\u0131lmay\u0131 engelleyecek, ikinci basamakta tedavi edilesi gereken hastalara yeterince zaman ayr\u0131lmas\u0131n\u0131 sa\u011flayacakt\u0131r. Aile hekimli\u011finin multidisipliner bir sa\u011fl\u0131k yakla\u015f\u0131m\u0131 oldu\u011fu d\u00fc\u015f\u00fcn\u00fcl\u00fcrse b\u00fct\u00fcnc\u00fcl bir sa\u011fl\u0131k hizmeti yakla\u015f\u0131m\u0131n\u0131 \u00f6ng\u00f6r\u00fcr. G\u00fcvene dayal\u0131 ileti\u015fim kurar, sorunlar\u0131 fiziksel, psikolojik ve sosyal y\u00f6nleriyle ele al\u0131r. Birey merkezli olmas\u0131n\u0131n yan\u0131nda b\u00fct\u00fcnle\u015ftiricilik, s\u00fcreklilik, aile ve topluma y\u00f6nelik olma \u00f6zellikleri nedeniyle aile hekimli\u011fi uygulamas\u0131n\u0131n \u00f6nemli bir yap\u0131 ta\u015f\u0131d\u0131r. Gittik\u00e7e artan orandaki ya\u015fl\u0131 n\u00fcfusun temel sa\u011fl\u0131k hizmeti gereksinimleri, onlar\u0131 tan\u0131yan ve kolay ula\u015fabilecekleri aile hekimleri vas\u0131tas\u0131yla \u00e7ok daha etkili olarak kar\u015f\u0131lanabilecektir.<\/p>\n<p>Birinci basamak sa\u011fl\u0131k hizmetlerinin etkili bir \u015fekilde verilebilmesi, toplumun hastal\u0131k y\u00fck\u00fcn\u00fcn azalt\u0131lmas\u0131n\u0131n yan\u0131 s\u0131ra ikinci ve \u00fc\u00e7\u00fcnc\u00fc basamak tedavi kurulu\u015flar\u0131m\u0131z\u0131n da daha iyi ve kaliteli sa\u011fl\u0131k hizmeti ve sa\u011fl\u0131k e\u011fitimi vermelerine f\u0131rsat tan\u0131yacakt\u0131r.<\/p>\n<p>D\u00fcnya Sa\u011fl\u0131k \u00d6rg\u00fct\u00fc, 2010 y\u0131l\u0131na kadar insanlar\u0131n aile ve topluma dayal\u0131\u00a0 temel\u00a0 sa\u011fl\u0131k hizmetlerine daha iyi ula\u015f\u0131labilmesini, Alma Ata kongresinde de al\u0131nan karar\u0131n devam\u0131 olarak 21. y\u00fczy\u0131lda \u201cHerkes i\u00e7in Sa\u011fl\u0131k\u201danlay\u0131\u015f\u0131n\u0131\u00a0 hedefleri aras\u0131nda saymaktad\u0131r. Bu hedefi ger\u00e7ekle\u015ftirmeyi\u00a0 ama\u00e7layan Sa\u011fl\u0131kta D\u00f6n\u00fc\u015f\u00fcm Program\u0131, aileye dayal\u0131 temel sa\u011fl\u0131k hizmetlerini gerekli e\u011fitim ve beceri ile donanm\u0131\u015f sa\u011fl\u0131k ekipleriyle, entegre bir \u015fekilde sunmay\u0131 \u00f6ng\u00f6rmektedir. D\u00fcnyan\u0131n pek \u00e7ok \u00fclkesinde aile hekimli\u011fi ba\u015far\u0131yla uygulanm\u0131\u015f olup \u00fclkemizdeki aile hekimli\u011fi uygulama \u00e7al\u0131\u015fmalar\u0131 desteklenmektedir.<\/p>\n<p>\u00dclkemizde 5258 say\u0131l\u0131 Aile Hekimli\u011fi pilot uygulamas\u0131 hakk\u0131nda kanun 24.11.2004 tarih ve 25650 say\u0131l\u0131 Resmi Gazete\u2019de, ard\u0131ndan s\u0131ras\u0131yla Aile Hekimli\u011fi pilot uygulamas\u0131 hakk\u0131nda y\u00f6netmelik 06.07.2005 tarih ve 25867 say\u0131l\u0131 ve Aile Hekimli\u011fi pilot uygulamas\u0131 kapsam\u0131nda Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131\u2019nca \u00e7al\u0131\u015ft\u0131r\u0131lan personele yap\u0131lacak \u00f6demeler ve s\u00f6zle\u015fme \u015fartlar\u0131 hakk\u0131nda y\u00f6netmelik 12.08.2005 tarih ve 25904 say\u0131l\u0131 Resmi Gazete\u2019de yay\u0131mlanm\u0131\u015ft\u0131r. Bu kapsamda 15.09.2005\u00a0tarihinde Aile Hekimli\u011fi Pilot Uygulamas\u0131na D\u00fczce ilimizde ge\u00e7ilmi\u015ftir. Halen\u00a081 ilimizde birinci basamakta Aile hekimli\u011fi modeli uygulanmaktad\u0131r. Aile Hekimli\u011fi uygulama y\u00f6netmeli\u011fi 25.05.2010 tarih ve 27591 say\u0131l\u0131 Resmi Gazete\u2019de yay\u0131mlanm\u0131\u015f ve bu y\u00f6netmelik 06.07.2005 tarih ve 25867 say\u0131l\u0131 Aile Hekimli\u011fi pilot uygulamas\u0131 hakk\u0131nda y\u00f6netmeli\u011fi y\u00fcr\u00fcrl\u00fckten kald\u0131rm\u0131\u015ft\u0131r. 2010 y\u0131l\u0131 sonunda t\u00fcm illerimizde Aile Hekimli\u011fi uygulamas\u0131na ge\u00e7ilmi\u015ftir.<\/p>\n<p>\u201c30.12.2010 tarihli ve 27801 say\u0131l\u0131 Resmi Gazetede Aile Hekimli\u011fi Uygulamas\u0131 Kapsam\u0131nda Sa\u011fl\u0131k Bakanl\u0131\u011f\u0131\u2019nca \u00c7al\u0131\u015ft\u0131r\u0131lan Personele Yap\u0131lacak \u00d6demeler ve S\u00f6zle\u015fme \u015eartlar\u0131 Hakk\u0131nda y\u00f6netmelik yay\u0131mlanm\u0131\u015ft\u0131r.\u00a0 Aile Hekimli\u011fi Pilot Uygulamas\u0131 Hakk\u0131nda Kanun 02.11.2011 tarihli ve 28103 say\u0131l\u0131 Resmi Gazete\u2019de yay\u0131mlanan \u201c11.10.2011tarihli ve 663 say\u0131l\u0131 KHK\u201d ile Aile Hekimli\u011fi Kanunu olarak ismi ve i\u00e7eri\u011finde de\u011fi\u015fiklik yap\u0131lm\u0131\u015ft\u0131r.
